5 Evergreen Instagram Reel Ideas Every Practice Should Keep on Repeat [Episode 178]

If you’ve ever found yourself staring at Instagram wondering, “What should I post today?” you’re definitely not alone.

One of the biggest mistakes I see practitioners make is believing they need a brand-new content idea every single time they open Instagram.

You don’t.

In fact, some of the best-performing content is built from the same proven frameworks over and over again. The key isn’t constantly reinventing the wheel—it’s taking an evergreen concept and making it specific to your ideal patient.

In this episode, I’m sharing five Instagram Reel frameworks that we keep on heavy rotation for our clients because they consistently work. You can reuse them again and again by simply changing the symptoms, audience, or solution you’re talking about.

Why Evergreen Reels Work

Evergreen content doesn’t rely on trends, holidays, or viral audio. Instead, it speaks directly to the problems your ideal patients are searching for every day.

That’s why these Reel ideas continue to perform months—or even years—after they’re first created.

The biggest mindset shift I want you to make is this: You have permission to reuse your best ideas. I do it all the time.

If you go look through my Instagram, you’ll notice I revisit the same concepts regularly. The wording changes, the examples change, but the framework stays the same because it works.

Reel Idea #1: Challenge a Common Assumption

One of my favorite Reel formats starts by questioning something people already believe.

For example: “Think the only way to heal your chronic shoulder pain is with medication or surgery?”

From there, introduce a specific number of alternatives, exercises, or strategies.

This format works because it immediately captures attention while positioning you as someone with a different perspective.

The important part is being incredibly specific about the problem you’re solving.

Reel Idea #2: Share What Makes Your Approach Different

Another framework I love starts with your experience.

Something like: “After 15 years in practice…”

Then explain a common belief you don’t agree with and why.

This isn’t about criticizing other providers. Instead, it’s an opportunity to educate your audience and explain your philosophy in a way that builds trust.

Reel Idea #3: Call Out Your Ideal Patient

Sometimes the simplest content performs the best.

Start by speaking directly to the exact person you want to reach.

“Calling all toddler parents…”

Or:

“Calling all runners…”

Then list a few specific symptoms or challenges they might be experiencing before inviting them to take the next step.

When people immediately recognize themselves in your content, they’re much more likely to stop scrolling.

Reel Idea #4: Tell People They’re in the Right Place

This framework is one of the easiest ways to attract local patients.

For example: “If you’re a marathon runner in Cincinnati looking for surgery-free knee pain relief, you’re in the right place.”

Notice how specific that is.

Instead of talking to everyone with pain, you’re talking directly to one type of person with one specific goal in one s

pecific location.

Specificity always wins.

Reel Idea #5: Offer an Alternative

This Reel format compares a common recommendation with your own approach. For example: “Instead of ibuprofen for headaches, may I suggest…”

Then share one or more alternatives that fit your philosophy and scope of practice.

It’s simple, educational, and naturally encourages curiosity.

Stop Reinventing the Wheel

If there’s one takeaway from this episode, it’s this: You do not need brand-new content ideas every week.

Find a handful of proven frameworks, tailor them to your ideal patient, and keep using them. That’s exactly how sustainable content marketing works.

Small changes in your messaging can make the exact same framework feel completely fresh to a new audience.

Molly: Hello, my loves. Um, I feel like I haven't done a solo episode in a long time. I said this in the intro of the last one, but if you didn't hear it, I'll say it again. I did, um, bump down to doing podcasts only every other week instead of weekly for the foreseeable future. I just have so many irons in the fire right now.

I was like, "Okay, what's one easy thing that can just kind of like take a pause?" And so this was, this was it. But I will very likely be back to weekly at some point. Um, I just have a... You know, right now the ducks, the ducks, there's no row, right? Like, there's no semblance of a row. The duck is like one duck is in China, one duck is on Mars.

So when I get my ducks back in a neat little line, I will come back to weekly podcasting. So today's episode is going to be very actionable, short and sweet, and I'm just giving you five reels that we always keep on heavy rotation for our clients that you can always have in your back pocket. Molly: Okay, these are the types of reels, these five reels that I'm gonna give you. Um, like I said, I would go ahead and grab that freebie that I said in the intro if you didn't hear it. These I would have on heavy rotation, and the reason I love these so much is because they're evergreen, these reels ideas or reels hooks. They're evergreen, meaning they always work. And if you message them in a way like I teach of being, like, uber specific in your messaging, then you could literally reuse these same, even the same hook over and over and just add in, like, a different problem, symptom, ideal client, whatever.

Like I said, these, like... Y'all gotta stop reinventing the wheel. It's, like, probably the biggest thing that I beat my head against a wall of, like... People are like, "Oh, my God, I can reuse that? You swear?" I'm like, "I swear. Go study my Instagram. You'll see how much I res- use stuff all the time." So version, uh, reel number one, um, let me give you the example version first versus the blank.

The example version is, "Think the only way to heal your chronic shoulder pain is with meds or surgery? Then check out these four alternatives before going under the knife." So the blank version would be, "Think blank," whatever the desired outcome, "is blank," too expensive, hard, whatever. "Then check out these," and you wanna have a specific number of, and it can be, like, products, hacks, or exercises.

So again, the example version, "Think the only way to heal..." And you could change this. "Think the only way to heal your toddler's chronic constipation was with MiraLAX? Check out these four alternatives instead," or something like that. Like, you can kind of modify it however you need. So hopefully you see how that reel...

I mean, this could just be used over and over and over. And you wanna be mega, mega specific, okay? Well, y'all know this. If you're new here, hello. I preach that you need to be so specific

Dr. Tara Carlson: to cut through the noise.

Molly: Reel number two, the example version. "After 15 years in practice, I'm the type of doctor who doesn't believe you have to accept an inability to conceive with, quote, 'normal' labs as, quote, 'unexplained infertility.'

Here's where conventional fertility guidance gets this wrong." That's a little wordy, I know, saying it, but once you see it on screen it's not bad. So the blank version for you is, "After blank years in practice, I'm the type of..." And you could do doctor, acupuncturist, chiropractor, massage therapist, whatever,

who doesn't believe you have to blank-" Like, so what's some common advice that they get that you just disagree with in order to blank, like have their desired outcome? Here's why most... And I, I don't really love a us versus them mentality, so you know, I, I'm not real huge fan of being like, "Here's where most OBs get this wrong," or something like that.

But you totally can if that's more of your personality, but you could do, you know, here's why, this is... That's why I wrote conventional fertility guidance or here's where conventional, um, you know, back pain guidance gets this wrong or, you know, whatever. Traditional YouTube stretching videos gets this wrong, like any of that stuff.

So that's number two. Number three, so easy. This one's so, so, so easy to repurpose a million different ways. I would have this one on rotation at least once a month, and the example version is, "Calling all toddler parents. If you're noticing your little one," and then there's like a colon and then there's four bullet points, but instead of bullet points we used emojis, "isn't pooping regularly, has trouble sleeping, is not meeting milestones, or is getting sick more frequently, then come see us in Raleigh, North Carolina."

So the blank version, calling all blank. So you just want to be like really, really, you know, specific of who your ideal client's... Women over 40 trying to conceive or, you know, whatever, um, you know, CrossFit athletes or calling all CrossFit athletes or calling all what- uh, you know, whatever. If you're noticing XYZ.

Now this, my example version has four bullet points, but depending on how long your bullet points are, you don't want to have like so much text on the screen that it's overwhelming, so you might just want to do three. Then come see us for a blank. So like, you know, whatever. In the example version, I just have, "Come see us in Raleigh, North Carolina."

But if you, like maybe you used to, you could say like, "Then come see us for a cupping treatment by our specialized acupuncturists in city, state," or something like that Reel number four, and again, go download this freebie so you can see them, mollycahill.com/reels or DM me Reels1. Here's the example version.

"If you're a mom in Raleigh looking for a MiraLAX-free solution for your child's constipation, you're in the right place." For some reason, I decided to put two examples on this. Here's the other one. "If you're a marathoner in Cincinnati looking for surgery-free knee pain relief, you're in the right place." So notice how I didn't just say pain relief, right?

I said surgery-free knee pain relief. Very different. Um, so the blank version is, "If you're a blank," so, like, you wanna look at, like... You know, I always like to use keywords of where you're located, "in blank." So if your ideal patient in whatever your city and, um, who wants desired outcome, you're in the right place.

And here is the last one. Here's the example version. "Instead of ibuprofen for headaches, may I suggest cupping, electrolytes, and magnesium?" Very simple. So what you're doing is you're putting, like, a really common recommendation first, and then you're giving, like, what your recommendations would be instead.

So instead of blank, like, common recommendation for blank, which is the undesired outcome, may we suggest blank, blank, or blank? Like I said, and you might not have three recommendations. You might only have one, or two, or you might have five. All of these also lend themselves to carousels if you wanna, like, remake them into carousels as well, but I just have them all as reels.
